Overview / Job Description

In short, we strive to provide optimal health across the lifespan for the populations we serve.
What You’ll Do:
Nurse-Family Partnership (NFP) is a national evidence based maternal, infant and early childhood home visitation program, which provides communities the opportunity to foster long-term success for pregnant and parenting clients, their children and families.  The program offers comprehensive, professional public health nursing services to low income, vulnerable, first time pregnant women and their families, from early in pregnancy to the child’s second birthday. Primary goals of the program include improved pregnancy outcomes, optimal child health and development, positive parenting and family self- sufficiency.
Using principles of supervision, reflective practice and staff development, the supervisor provides primary oversight and support to 8 NFP nurse home visitors, building and modelling a strengths-based client centered culture of nursing excellence. This integral role ensures program operations align with NFP program fidelity measures, TCHD policies and procedures and grant expectations.  Travel is required throughout Adams, Arapahoe, Denver and Douglas Counties and to other locations in the Denver Metro area.  Staff will be required to work at their designated agency office location and will need a HIPAA secure home location which complies with agency telework expectations.
Essential duties of this nursing leadership role:
  • Recruits, hires and orients nursing staff, ensuring completion of all required NFP model education and attainment of NFP required caseloads. Evaluates staff performance.
  • Assesses knowledge and skill gaps and facilitates professional development and coaching opportunities to support the achievement of NFP nursing competencies and excellence in nursing practice.
  • Provides weekly reflective supervision and assists nurses in analyzing complex problems and case situations, planning and delivering quality client care.
  • Actively participates in the preparation, monitoring and expending of the program budget and compliance with contract deliverables.
  • Utilizes data to monitor implementation efforts and develop quality improvement strategies.
  • Promotes awareness of NFP program and engages community partners to build and sustain referral networks and systems of care for prospective and enrolled clients.
  • Creates a team culture that engages staff and supports nurse and client retention.
  • Serves as an integral member of the NFP leadership team which guides overall implementation of the NFP Program and supports the broader efforts of the TCHD Nursing Division.
Requirements You’ll Need:
  • Bachelor’s Degree in Nursing from a school accredited by the National League for Nursing Accrediting Commission or the American Association of Colleges of Nursing.
  • Master’s Degree in Nursing or a closely related field preferred.
  • Requires a minimum of three years progressively responsible public health nursing experience, including supervisory  experience.
  • Experience in NFP or public/community health maternal child home visitation (highly desired)
  • Active Colorado Nursing license and Current CPR certification
  • Valid Colorado Driver’s license in good standing.
  • Access to reliable vehicle and automobile insurance.
  • Comfortable with regular use of an automobile, travelling and working in the community, with unpredictable weather and less than optimal environmental conditions at times.
  • The ideal candidate will be able to quickly develop rapport and trust with staff who support a broad range of diverse clients, demonstrate flexibility, adaptability and creativity in delivery of nursing supervision, employ strong computer and technology skills and thrive in both traditional and virtual team environments.
