Overview / Job Description

This position is eligible for a $10,000 sign on bonus. and relocation.

The sign-on bonus will be paid out in two installments: the first pay check and after completion of one year of satisfactory employment.

Please note that current University of Vermont Health Network employees are excluded from eligibility for this bonus.

About The Post Acute Unit at Helen Porter Rehabilitation & Nursing

  • Renovated 27-bed unit
  • Average length of stay: 20 days
  • Rehab-focused care with in-house rehab therapists

Principal Responsibilities and Tasks

  • Rotating call coverage.
  • Assists in establishing and implementing standards which ensure safe and therapeutically effective nursing care of residents.
  • Participates in the establishment and promotion of administrative policies and practices which favor the practice of nursing and provide a climate which helps nursing personnel to increase their professional, technical, and psychosocial skills.
  • Monitors overall supervision and evaluation of care of nursing unit. Assures total needs of the resident are met and that the quality of service is in accord with the standards set forth by the facility.
  • Ongoing monitoring of the residents health record to assure accuracy, completeness and timeliness of required documentation.
  • Assists with and implements a staffing plan which promotes stated objectives and standards of nursing practice and the maximum utilization of nursing personnel.
  • Evaluates nursing care and the climate in which it is practiced to identify accomplishments and problems for purposes of forecasting and planning.
  • Supervises all nursing activities and promotes improvement in nursing care.
  • Assists staff/charge nurses in problem solving and reports same to DNS.
  • Assists with on call coverage for nursing on weekends and holidays and off shifts.
  • Assists in the development and maintenance of an effective system of documentation and reporting by nursing personnel:
  • Interprets, supports, and enforces nursing home policies and procedures.
  • Attends meetings and conferences as requested.
  • Is aware of potential safety risks and monitors nursing staff to maximize safety within the facility. Participates in fire drills.
  • Participates on committees as assigned.
  • Manages emergency situations involving residents/visitors and personnel.
  • Serves as role model for other nursing staff members.
  • Observes facility policies and procedures regarding attendance, timeliness, house rules, teamwork, safety, customer service and other policies/procedures that may be introduced from time to time.

Job Requirements
Education

  • Graduate from an accredited school of nursing (RN program)
  • Current RN licensure or compact licensure recognized by the State of Vermont required.

Experience

  • Two or more years of nursing experience in Long Term Care.
  • One year of administrative or supervisory experience.
  • Advance preparation of Unit Management and principles of supervision desirable.
  • Experience in the field as a professional nurse with supervising skills utilized.
  • Knowledge of and interest in field of geriatrics essential.

Knowledge/Special Skills

  • Establishes and maintains a good relationship with physicians, other departments, and all other staff.
  • Demonstrated ability to supervise, teach and to assist others in improving their skills.
  • Good understanding of state and federal regulations.
  • Ability to expertise good judgment and use initiative in determining the appropriate course of treatment in order to assure optimum physical, mental and psychosocial well-being of every resident.
